City officials to vote on street, van plans

Exactly two weeks after a “road diet” plan was unveiled for North Main/Renfro streets, city officials are scheduled to decide if it will be on the menu for Mount Airy motorists.

The proposal by the N.C. Department of Transportation would reduce the lanes of travel from four to two in each direction — while adding a center turn lane and bike lanes on either side of the roadway. It has arisen in connection with the upcoming resurfacing of the portion of that corridor running from Lebanon Street to Independence Boulevard.

Such a format has the name “road diet” because the size of the roadway is being compressed, with traffic slowed down as a result.

These changes are best implemented when repaving occurs, DOT officials say, since they can be incorporated with the re-striping that is required.

Eventually, the same configuration for the now-four-lane corridor could be extended south along Renfro and Main streets to the Ararat River bridge at Bannertown.

But that is only if local officials give their authorization to the plan, a vote on which is scheduled during a Thursday meeting of the Mount Airy Board of Commissioners. It is scheduled to begin at 2 p.m. in the council chambers of the Municipal Building.

Also slated at Thursday’s meeting is a decision regarding a circulator route, or van shuttle service, that an area public-transportation agency is proposing to operate in Mount Airy. It has drawn fire from local taxicab operators.

“Road Diet” Indigestion?

Since the road diet proposal was unveiled at the commissioners’ last meeting on July 19, city officials report receiving many comments from citizens about the plan.

“The road change, I’ve heard tremendous feedback on that,” Commissioner Dean Brown, the senior member of the city board, said Monday.

“All the feedback has been against making single lanes where the double lanes are,” said Brown, who had expressed concern about the proposal on July 19.

He said then that older drivers coming from the north end of town often travel at slow speeds, even 12 miles per hour, but other motorists can now pass them when they reach the intersection of North Main and Lebanon streets.

This would not be possible if the road diet plan is put into effect.

“Still, I think a lot of people don’t understand what they are trying to do,” Brown acknowledged in reference to the intent of DOT officials.

A similar change was implemented earlier along East Atkins Street, which leads into downtown Dobson from U.S. 601. As a result, accidents have been reduced, a DOT representative has said.

One goal of the plan for Mount Airy would be improved safety for pedestrians at First Baptist Church and Reeves Community Center.

Transportation studies have shown that the overall traffic volume of the North Main/Renfro corridor has dropped over the years, apparently a result of economic factors such as local plant closings.

While believing it to be well-intended, Brown thinks problems would result from the change at peak periods, such as when school traffic is on local roadways in the mornings.

“They haven’t checked traffic at school times,” the commissioner said of DOT counts of vehicle volume. “They’re just doing average numbers, and the numbers are lower — but when the streets are busy, they’re busy.”

Van Plan Vote

Also on Thursday’s agenda is another transportation-related item for the city commissioners’ consideration. It involves a letter of support requested from the municipal government for Yadkin Valley Economic Development District Inc. (YVEDDI) to seek grant assistance from the N.C. Department of Transportation.

The transportation division of YVEDDI wants to use the money to launch a “circulator” route that would utilize a 19-passenger van to transport riders to key locations in Mount Airy at $1 per trip for the general public.

It would include a series of regular pickups at locations such as elder housing and low-income neighborhoods and stops at Walmart, medical facilities, city parks and other strategic sites. One would be the park-and-ride lot of a regional bus service, thus allowing users of the YVEDDI program to also access destinations such as Winston-Salem.

However, some local citizens — including cab operators — say the government-subsidized transportation service would put their private businesses at a competitive disadvantage and force them to close.

YVEDDI officials, on the other hand, contend that the circulator route would help people needing such a program who “typically do not use the taxi services because of the cost.”

“I can see both sides of that situation,” Commissioner Brown said Monday, acknowledging that there are people in Mount Airy who lack transportation.

“I understand the dilemma with the taxi drivers, and I understand what YVEDDI is trying to do.”

New Diet Pills Offer Option To Off-Label Obesity Drugs

Piper Miguelgorry started gaining weight in her 30s and 40s, after having three sons. The environmental researcher from Folsom, Calif., tried many diet regimes, without success, until she found a weight-loss center in nearby Roseville, where phentermine, a prescription appetite suppressant, was part of the plan, along with a high-protein, low-carb diet.

“Much to my surprise, I lost 50 pounds,” says Mrs. Miguelgorry, 58, who has kept it off for more than a year. She also beat Type 2 diabetes and cut her cholesterol in half. She credits phentermine with helping reduce her craving for carbohydrates and plans to stay on it indefinitely.




Two diet pills just approved by the FDA, Qsymia and Belviq, won’t be available for several months, but some doctors have been prescribing the ingredients of Qsymia (a migraine drug and phentermine, an old diet pill) to patients for years specifically for weight loss. Physicians who treat obesity hailed the Food and Drug Administration’s recent approval of two new diet drugs—the first in 13 years—as a new era in weight-loss management. But some obesity specialists, including those at prestigious medical centers, have been prescribing medications to help patients lose weight for decades, both on and off-label.

Those medications include phentermine and several other stimulants approved for weight loss in the 1950s, as well as drugs to treat diabetes, depression and attention-deficit disorder that have weight-loss as a side effect.

One of the newly approved drugs, Qsymia, is a combination of phentermine and topirimax, a drug for easing seizures and migraines that also tends to make people feel full. The other, Belviq, works on brain receptors for serotonin, a neurotransmitter than triggers feelings of satisfaction.

Physicians can legally prescribe medications for uses not specifically approved by the FDA. But most obesity doctors do so cautiously given the history of diet pills removed from the market due to serious side effects including death.

“The fact that we’ve got two new drugs approved will reaffirm to the public that this truly is a medical problem and there’s a place for pharmacotherapy,” says Ed Hendricks, head of the Center for Weight Loss Management, which treated Mrs. Miguelgorry.

While some experts argue that people who are obese should simply eat less and exercise more, many physicians who treat obesity say it is driven by biological changes in the body, not lack of willpower.

“People say, ‘Why are you treating a lifestyle problem with medication?’ If you think that, you don’t understand obesity,” says Louis Aronne, director of the Comprehensive Weight Control Program at New York Presbyterian Hospital/Weill Cornell Medical College. He says there is mounting evidence that in some people, neurons in the hypothalamus that should recognize when the body has stored sufficient fat and either cut food intake or boost energy expenditure aren’t working properly. Medications can help, says Dr. Aronne, who oversaw clinical trials of Qsymia.

In trials of 3,700 obese and overweight patients, those who took Qsymia for one year lost 6% to 9% more of their body weight than those on a placebo. Nearly 70% lost at least 5% of their weight.

With Belviq, of nearly 8,000 patients studied, those on the drug lost about 4% more than those on placebo. All were also counseled to exercise and eat a reduced calorie diet. The FDA says people who don’t lose at least 5% of their weight in 12 weeks should discontinue the drugs.

People who eat very large portions or think about food all the time, even right after a meal, aren’t getting biochemical signals to stop eating and may find drugs helpful, says Robert F. Kushner, clinical director of the Comprehensive Center on Obesity at Northwestern University.

People who eat for emotional reasons tend to benefit less, although drugs can help clarify their motivations. “If you give a medication that makes them feel full, but they still keep eating, it becomes very clear that it really is emotional,” says Dr. Kushner, chairman of the American Board of Obesity Medicine.

The new drugs are FDA approved for patients with a body mass index of 30 or higher, or with a BMI of 27 and high blood pressure, diabetes, high cholesterol or sleep apnea. People with very large amounts of weight to lose may benefit more from bariatric surgery, or a combination, doctors say. The drugs aren’t covered by most insurance companies. They are expected to be available this fall.

Most of the drugs currently prescribed for weight loss suppress appetite by affecting neurotransmitters, which send signals in the brain. Stimulant medications are among the most powerful. The FDA approved several stimulants—including phendimetrazine (marketed as Bontril) and diethylpropion (Tenuate)—specifically for weight loss in 1959. They remain on the market today.

How They Work

Drugs sometimes prescribed for weight loss work in various ways.

Stimulants rev up the central nervous system.

Antidepressant drugs such as buproprion (Wellbutrin) affect brain chemistry.

The diabetes drug metformin (Glucophage) reduces the impact of blood sugar. Liraglutide (Victoza) boosts a gut hormone to promote fullness.

The FDA-approved obesity drug orlistat (Xenical, Alli) blocks fat absorption.

Naltrexone (Depade, ReVia) blocks opiod receptors and makes food less appealing.

The best known is phentermine, half of the popular fen-phen combination in the 1990s until fenfluramine was linked to heart-value damage and discontinued in 1997. Because they are potentially addictive, phentermine and the others are approved for short-term use only. “But all of us use it long-term,” says Dr. Kushner. Dr. Hendricks, who says he has treated 15,000 patients with phentermine, alone and in combinations, over 23 years, and says that patients who keep taking it tend to maintain their weight loss. Drs. Aronne, Hendricks and Kushner all consult for drug makers.

ADHD drugs including Ritalin and Adderall also suppress appetite in some patients, but most obesity doctors use older generic stimulants. Patients with heart disease, high blood pressure, hyperthyroidism or glaucoma should not use stimulants.

The diabetes drug Metformin helps decrease the amount of sugar the body absorbs from food and increases the body’s response to insulin. Some patients can experience dangerously low blood sugar.

It is unclear why bupropion (Wellbutrin) doesn’t make patients gain weight the way other antidepressants do, but it may help ease cravings. (Last month, GlaxoSmithKlein agreed to pay a $3 billion fine in part for promoting Wellbutrin for weight-loss and other off-label uses.)

Naltrexone (Depade, ReVia) helps alcoholics stop drinking by blocking opiod receptors in the brain, diminishing the kick booze provides. It may make food less rewarding the same way. Side effects, in rare cases, may include liver damage.

Some clinics promote human chorionic gonadotropin (hCG), an infertility treatment, as a way to “reset metabolism” when combined with a 500-calorie-a-day diet. But obesity experts say there is no evidence that hCG works better than a placebo. The FDA has warned seven companies to stop selling over-the-counter hCG and making unsupported claims.

Respected obesity experts say consumers should be wary of purported diet drugs sold over the Internet, as well as doctors who sell drugs from their offices, with little or no evaluation or without a comprehensive weight-loss plan.

We recently had the opportunity to speak with U.S. Olympic Committee Nutritionist, Jennifer Gibson, Msc, RD, IOC DIP, who has been preparing many of the Team USA’s top athletes for competition in London this summer.

I work with an interesting population: the combative and acrobatic sports. That means wrestling, fencing Tae Kwon Do — things with weight classes that require managing weight really closely.

On the acrobatic side, it’s diving, gymnastics, synchronized swimming, all of which require a lean physique and light body weight.

For most of my athletes, weight control is all about pretty restrictive diets. Basically, everything they put in their mouth has to be monitored and thought about. We usually use a phased diet approach. Six to eight weeks out, we change from the initial diet to one with more careful considerations. They can choose to go on strict plans with portions meted out, or they can just get serving guidelines and build their own meals.

Some athletes need to make a few behavioral changes: if they’re evening snackers, that’s something we phase out, for example.

Once we get to the restrictive diet, it’s an extreme situation: it’s not just about fitting into a dress. These guys have to be on the scale number or they don’t get on the mat. First, we’ll do an artificial weight loss from sweating out water weight. Then maybe a liquid diet for a few days. It’s an extreme world, because you have to be so precise on the scale. But these diets zoom in and out — there are periods of coming back to a more liberal, less detailed approach on diet.

By contrast, with the aesthetic sports, it’s an ongoing, close attention to diet because they can’t afford to put on any extra weight or fat mass. You’re trying to propel your body through air — dead weight will have an impact.

We create a strict daily diet. Even though there are off seasons, most of them train and compete year round. They have a whole food-based diet that’s very, very natural: regular meats, fresh fruits, nuts and seeds, veggies. These aren’t processed foods. That’s because it’s mainly the quality of food that’s how they’re getting their nutrients.

What we know is that you can get the same nutrition from convenience foods, but it won’t be of the same quality. The quality of the protein you get from chicken breast is just higher than what you’ll get from a powder. Their bodies are their jobs, so how they feel really determines how well they’ll succeed.

A lot of the athletes aren’t using supplements because they have to be careful from a doping perspective — you don’t really know what’s in a lot of them. There have been cases of supplements containing banned substances. This is starting to be regulated and it’s getting better, but it isn’t guaranteed.

With real food, they know what it is and what the quality of the nutrition will be. Of course, athletes will use bars, powders and gels, especially for convenience while traveling, but it isn’t the main resource we use.

I’d say that Olympic athletes suffer from many of the same challenges and problems that regular people face when it comes to nutrition and diet. Just like at home, here they have a lot of choice and variety: there’s an ice cream machine. They struggle with temptation and boredom eating. When they aren’t at a training facility, they’re living on their own, cooking and shopping. Between jobs and training, they can be sometimes too exhausted to plan their eating right — that’s very similar to a lot of us.

Also similar, most of them are on a strict budget. In amateur sport, you aren’t making a lot of money. And they also travel for work: they’re navigating changing time zones and cultural food environments.

There are differences, too, mostly related to training. If they train too hard, for example, they lose their appetites. So if it’s a high volume training block or week and it’s very strenuous, they’ll struggle to eat. On the other side, much of training makes you hungry, so they also struggle with increased hunger.

But here are some takeaways for everyone:

Emotional Eating
All they’re doing is eating, sleeping, training — there’s a lot of down time. So they’ll eat out of boredom, stress, comfort, anxiety … a lot of emotional things. The key there is to just be really self-reflective and aware of what your relationship to food is. Once they find the trigger, we try to divert their habit a bit; if it’s anxiety, for example, we’ll try breathing techniques. If it’s boredom, it’s about finding something else to fill the breaks in between training sessions. A lot of athletes are night snackers, so we do evening walks to break the habit.

An evening walk is almost like walking a dog without the dog. You just put your iPod on and go for a walk, just to change the behavior about. We work around emotionally charged habits. We ask the athletes: are you physically hungry or are you psychologically hungry? Sometimes the hunger is a feeling that you should eat: is it because you did your cardio and feel like you deserve something?

If it’s truly physical hunger, we adjust when the athletes are snacking, talk them through hydration to make sure that isn’t causing the hunger. We also check in about planning throughout the day. If you’re hungry at 4 p.m. because you had a skimpy lunch, then we know how to change things to fuel for activity.

Poor Planning
It’s important to avoid really dire hunger situations. Once you get to that point, you’re more likely to overeat and make poor choices. If you’re hungry, your body’s telling you something. A balanced snack is something that’s high in protein: Greek yogurt, for example. Or trail mix and milk, that’s a blend of carbs. Or maybe for that day, it makes sense to bump up your lunchtime. Just avoid driving yourself into the deep hunger zone.

Get Personal
If you want to get serious about nutrition, it’s important that you seek help of nutrition professional. Making that small investment in yourself will help so much more than those generalized approaches. Most registered dietitians will have different counseling packages. And obviously, a person’s individual needs dictate the duration of counseling. If you need help with accountability, you might need ten sessions. If you just want some basic information and you’re pretty self motivated, you can get away with just one or a few sessions.

Often times, people will spend money on supplements, books, and prepackaged food. That’s cool, but you’re spending the money anyway. You might as well go to the people who are trained to do this and get that individualized programming.

Be Realistic
A lot of times, when they take up an exercise program, people expect to lose weight and don’t. I’ve known people who gained weight while training for marathons. It’s all about appreciating how many calories you are really burning. Exercise is always great. But if you’re going for a 30-minute run, that’s not 1,000 calories. The Olympic athletes I work with are training four, size, sometimes eight hours a day. That’s exponentially higher than the average person because their job is to be an athlete. If you’re exercising, fantastic. If you’re exercising for weight loss, you don’t need to eat more. It’s the calorie deficit you create by working out that will help.

Eat Real Food
Going to food first is not only possible. It’s a good idea. The highest achieving athlete, who is training eight hours a day, 95 percent of his diet is whole and real foods. You don’t have to lean on potions or pills. Food is the best source of nutrients. It’s not as sexy as some new seed from the Amazon, but 99 percent of the time, people get their nutritional material from food. You might need to supplement, but if these athletes can do with food, so can you.

Texans’ Arian Foster insists he knows best about his vegan diet in face of criticism

“I had a long conversation with him about that. I told him, ‘If this doesn’t work, I’m going to kick your ass,’ ” teammate Brian Cushing said. “I told him that because he’s going too far. He thinks he knows more than me, but he doesn’t, especially about nutrition. We have a good relationship, but I told him this better be right. We have a lot riding this year.”

Foster hears that comment, nods slowly and smiles. He also has had a detailed discussion with general manager Rick Smith about the dietary choice. In the one month since he announced his departure from an appetite for flesh, he has seen an absurd amount of interest.

“Everybody cares what I eat now,” Foster said. “They didn’t care before, but they do now. Everybody is a nutritionist now and they’re an expert on protein. Every day, every single day somebody knows something new to do. I just smile and say, ‘OK.’ ”

Adding to the fear is that Foster has some strong similarities with the likes of Ricky Williams, from his NFL counterculture diet (Williams became a vegetarian in 2005) to his deep thinking. Even the cadence of Foster’s speech is reminiscent of Williams. More important than any of that is Foster, like Williams, is supremely talented. Between his hard-running style (he has back-to-back 1,000-yard years) to his receiving ability, Foster is one of the best all-around backs in the league – and he’s paid that way, too.

Despite a hamstring injury that forced him to miss the better part of the first three games, Foster ended up with 1,224 yards rushing, 53 receptions and 12 total touchdowns. Foster helped Houston get to the second round of the playoffs, even though the team was down to then-No. 3 quarterback T.J. Yates by the end of the season.

Now, Foster has people worried that he’s messing with a good thing. Foster believes he’s creating a healthier, stronger body that will make him a better player. He consulted doctors before choosing this path, which is more restrictive than a vegetarian diet. As it was, he had gotten down to eating red meat roughly once every six months. His logic was simple: He didn’t feel that good when he ate a big meal that featured meat.

“I didn’t just blindly stop eating meat. I know what I’m doing,” Foster said.

“I saw a documentary in high school that really turned me on to getting aware of it. It didn’t change my diet then, but it made me think about the myths about protein. I said a while ago that when I quit playing football, I would probably become a vegetarian and I thought I needed the protein. Then I did some research with doctors who in their world are considered kind of radical. To me, it’s radical we have heart disease and 12-year-old kids with diabetes. ”

That’s typical Foster. As a child, his parents pushed him to think deeply about subjects.”Question everything. Don’t take anything at face value. Find your own path and come to your own conclusions,” Foster said, mimicking his parents.

That sense of curiosity got Foster into trouble. At the University of Tennessee, Foster admitted that he butted heads with the coaching staff when he was required to go to every class, even sending monitors to make sure he went.

Arian Foster’s choice to go vegan has been one of the hot topics this NFL offseason.

“I’m 19 years old, I can get myself to class,” Foster said of that time. To him, as long as he did the work and got the class notes, he felt he should be afforded the same freedom as other students.

“The professors would tell the students that they didn’t have to go to every lecture, but not the football players. We had to go to every class,” Foster said. As a result, Tennessee coaches ripped Foster to NFL coaches and personnel men, helping him go undrafted.

Fast forward to this season and you have greater concern about what’s at stake. That’s why Smith, who has also given up eating red meat on a regular basis, had the meeting with Foster.

“We want to support him and make sure that he has the guidance and help to get through the things he has to deal with in the season,” said Smith, who negotiated a five-year, $43.5 million extension with Foster this offseason.

Foster appears to have plenty of that. He also has history on his side. Williams, for instance, had five relatively successful seasons, including 1,121 yards in 2009, after becoming a vegetarian.

A better example is Atlanta tight end Tony Gonzalez, a likely Hall of Fame player, who became a vegetarian earlier in his career when he played for Kansas City. Although Gonzalez returned to eating chicken and fish, he has every major receiving record for tight ends.

Houston trainer Cedric Smith also worked with Gonzalez when they were with the Chiefs, giving him experience on that front.

“The one thing that I remember is that [Gonzalez] did lose weight at first, but it came back pretty quick,” Cedric Smith said. “He worked his way around it; he worked with the nutritionist there, Mitzi Dulan, and they came up with a plan. He had a plan to do what he needed to do. I think he bent in some areas to form the diet that he needed and he wanted, but he is mostly vegetarian. What I saw out of him is that he was the same player that he was before he did it.”

Foster appears to have a strong plan in place. Now, it’s a matter of dealing with the reaction. Foster’s curious nature has helped him intellectualize the issue.

“You start to understand why people are like that. We’re emotionally attached to food, bad food. Think about every big event in America, it’s attached to food. Christmas, Thanksgiving, birthdays, holidays … it’s with food. That’s why people feel so strongly about it; they’re emotionally attached to it.

“I know that’s why so many people are interested. They’re speaking from their heart … [but] I’m concerned with what I’m putting in my body. That bad stuff we eat, it goes into your body and it stays there. It stays in your heart. Trust me, I love that kind of food. I see a hamburger and I know it’s delicious. But now I can think it through and know that hamburger is not necessarily good for me.”
